Please download, complete, and return all required forms before your first session. You may email them to tim@timdallacqualcsw.com or bring them to your first appointment.

Before Your First Session

  1. Download the intake packet below.
  2. Complete all forms thoroughly — take your time with the history sections.
  3. Return completed forms via secure email to tim@timdallacqualcsw.com at least 24 hours before your first session.
  4. If you have questions about any form, contact Tim at (406) 564-1848.
  5. By completing and returning these forms, you acknowledge receipt of our Privacy Policy.

Texting & Email Consent Forms

If you plan to communicate with Tim by text message or email, please download, sign, and return the appropriate form below. These forms explain the privacy limitations of each method and your options for more secure communication.

Text Messaging
Consent to Use Text Messaging
Acknowledges that SMS text messaging is not HIPAA-compliant and may not be fully private. Required if you wish to communicate with Tim by text for scheduling or brief logistical messages.

Consent to Use Email Communication
Acknowledges that standard email is not fully HIPAA-compliant. Includes the option to use Proton Mail — an encrypted, more private alternative — at no cost. Required if you wish to communicate by email.
