Please download, complete, and return all required forms before your first session. You may email them to tim@timdallacqualcsw.com or bring them to your first appointment.

Before Your First Session

  1. Download the intake packet below.
  2. Complete all forms thoroughly — take your time with the history sections.
  3. Return completed forms via secure email to tim@timdallacqualcsw.com at least 24 hours before your first session.
  4. If you have questions about any form, contact Tim at (406) 564-1848.

Tim is a private-pay provider. Session fees are discussed during the initial consultation. A sliding scale may be available based on financial need.
